Tasks

The aim of the social service is to provide advice, support and organization of help using and taking into account the patient's existing resources and the social environment. In doing so, an individually tailored, short, tailor-made, person-centered help should be found that is appropriate to the personal and social situation.

Our tasks in the context of psychosocial counseling include: 

  • Organization and clarification of costs for medical rehabilitation (e.g. follow-up rehabilitation, early rehabilitation, geriatric rehabilitation)
  • Advice and application for inpatient nursing care
  • Advice on palliative care
  • Advice and application for benefits under the Severely Disabled and Care Act
  • Advice on care, power of attorney, advance directives
  • Advice on necessary and additional help before discharge, such as: B. Aid supply, home nursing
  • Psychosocial interviews for cancer and chronically ill patients
  • Information and, if necessary, mediation to self-help groups and associations
  • Discussions with addicts and referral to specialized services and advice centers
  • Advice on personal, family, economic and legal questions
  • Advice and interventions in emergencies
