Dear patient,
The best possible care during your inpatient stay in our clinic is very important to us. Even after the hospital treatment in our facility has been completed, we will continue to take care of your complete follow-up care as part of a nationwide discharge management, if this is necessary.
With our new, optimized discharge management, we ensure seamless follow-up care for you, which can be done on an outpatient basis, in inpatient rehabilitation or in the nursing facilities. For this purpose, we will determine - subject to your consent - at the beginning of your stay whether and, if so, which measures should be initiated immediately after your inpatient stay. Of course, we take your individual needs into account as much as possible. If a follow-up care is necessary, we will organize it accordingly and in close consultation with you and your health / long-term care insurance.
In order to implement our new discharge management together with you and to be able to respond to your individual needs in the best possible way, we need your support. During your inpatient admission, our employees will collect the necessary information that is crucial for your ideal follow-up care.
